My new job is going pretty well. I really admire and like the psychologists I work for. I have been there for almost a month now and am finding it to be quite fascinating. I do worry though that a patient might come in someday and recognize me (Seattle is a very small city).
This job has also given me some great insight into sexuality . . . what is perceived to be normal/healthy and what is not. The other day, I picked up and started reading the DSM manual, which describes various psychological conditions and provides their CPT codes for insurance purposes. I found the section which covered sexual problems, entitled, "Paraphillias." The section read:
302.4 Exhibitionism
A. Over a period of at least 6 months, recurrent, intense sexually arousing fantasies, sexual urges, or behaviors involving the exposure of one's genitals to an unsuspecting stranger.
B. The fantasies, sexual urges, or behaviors cause clinically significant distress or impairment in social, occupation, or other important areas of functioning.
302.81 Fetishism
A. Over a period of at least six months, recurrent, intense sexually arousing fantasies, sexual urges, or behaviors involving the use of nonliving objects (e.g., female undergarments).
B. The fantasies, sexual urges, or behaviors cause clinically significant distress or impairment in social, occupation, or other important areas of functioning.
C. The fetish objects are not limited to articles of female clothing used in cross-dressing (as in Transvestic Fetishism) or devices for the purpose of tactile genital simulation (e.g., a vibrator).
302.89 Frotteurism
A. Over a period of at least 6 months, recurrent, intense sexually arousing fantasies, sexual urges, or behaviors involving touching and rubbing against a nonconsenting person.
B. The fantasies, sexual urges, or behavior cause clinically significant distress or impairment in social, occupational , or other important areas of functioning.
302.83 Sexual Masochism
A. Over a period of at least 6 months, recurrent, intense sexually arousing fantasies, sexual urges, or behaviors involving the act (real, not simulated) of being humiliated, beaten, bound, or otherwise made to suffer.
B. The fantasies, sexual urges, or behavior cause clinically significant distress or impairment in social, occupational , or other important areas of functioning.
302.84 Sexual Sadism
A. Over a period of at least 6 months, recurrent, intense sexually arousing fantasies, sexual urges, or behaviors involving acts (real, not simulated) in which psychological or physical suffering (including humiliation) of the victim is sexually exciting to the person.
B. The fantasies, sexual urges, or behavior cause clinically significant distress or impairment in social, occupational , or other important areas of functioning.
302.2 Transvestic Fetishism
A. Over a period of at least 6 months, in a heterosexual male, recurrent, intense sexually arousing fantasies, sexual urges, or behaviors involving cross-dressing.
B. The fantasies, sexual urges, or behavior cause clinically significant distress or impairment in social, occupational , or other important areas of functioning.
302.82 Voyeurism
A. Over a period of at least 6 months, recurrent, intense sexually arousing fantasies, sexual urges, or behaviors involving the act of observing an unsuspecting person who is naked, in the process of disrobing, or engaging in sexual activity.
B. The fantasies, sexual urges, or behavior cause clinically significant distress or impairment in social, occupational , or other important areas of functioning.
302.9 Paraphilia Not Otherwise Specified
This category is included for coding Paraphillas that do not meet the criteria for any of the specific categories. Examples include, but are not limited to, telephone scatologia (obscene phone calls), necrophilia (corpses) partialism (exclusive focus on part of the body), zoophilia (animals), coprophilia (feces), klismaphilia (enemas), and urophilia (urine).
After reading this, I started thinking about some of the customers I've had.
I started thinking about the old guy with wild hair and wild eyes who comes into see me sometimes. Before he comes into the booth, he trys to masturbate in the bathroom (with the door open) or down the hallway, while he watches me. He tells me that his biggest fantasy is to be seen masturbating in public.
Then, there are the handful of customers who like to talk about poop and pee and how they want to see it come out, taste it, and have it all over them. Or the customer who looks totally normal, but wears a diaper under his pants.
I think about the 40-something year old guy with glasses and thin, stringy, black hair who comes in and wants you to tell him his cock is too small and you wouldn't fuck him if he was the last guy on earth.
Or . . . the guy who comes in with a rubber band around his balls so tight his cock's bulging purple.
Then there's the guy who goes from booth to booth, trying to find the girl with the loosest pussy. He wants to know if you've had any kids. Wants to look way up inside you . . . even brings a flashlight.
And then there's Japanese Panty Man who drops about $200.00 in the booth each time he comes in. He loves panties . . . wants you to wear the pair you wore all day - even if they're the white, full bottom, granny panty style.
I think about the sweet and shy customer I helped last month. He looks like Richard Gere. He fantasizes about wearing women's panties and stockings, but never has because he's married, and doesn't want his wife to know. He bought some panties and stockings downstairs and then wore them during our show. He loved every minute of it and thanked me over and over again for helping him.
Then, there's the guys who are married to women, but come into the booth talking about fucking guys. There's the guys who come in because their wives don't want to have sex, or they aren't into their fantasies. Then, there's the guy who doesn't give a fuck about his wife and he's doing "God knows what" behind her back.
I think about these people and I wonder what happens to them when they leave. Does their behavior "cause clinically significant distress or impairment in social, occupational, or other important areas of functioning"?
I'm certainly not so naive as to believe that all guys who go to peep shows suffer from some sort of sexual dysfunction. The majority of my customers seem to be pretty normal guys. But I do, now more than ever, wonder why they come and if they have to come.
I wonder if I've been too idealistic about my job.
I'm definitely not turning into a prude . . . I just always hoped that I was helping people.
I think it's interesting to note the language in part "B." of the aforementioned diagnosis, which reads, "the fantasies, sexual urges, or behavior cause clinically significant distress or impairment in social, occupational , or other important areas of functioning."
These traits are present, in addition to the actual behavior described in part "A," to be considered for such diagnosis. So I wonder . . . how much does our culture or belief system about sexual norms (stemming from religious ideologies, ignorance and fear) contribute to the pain and "significant distress" of others whose sexual needs and desires go beyond these norms. Further, is it possible that these beliefs exacerbate the condition to the point where people do eventually harm themselves, or others?
If society was more accepting of extraordinary sexual behaviors (behaviors that do not hurt themselves or others) would there be less sexual dysfunction overall? Or, like so many fear, would this acceptance lead us down a moral slippery slope, causing people to lose their minds and good judgment.
